No, unless stated in your employer guide. Contributions are optional.
Eligible employees and their dependents may be covered. Dependents include a lawful spouse and children through age 25. Parents are not eligible.
Coverage typically begins on the first day of the month after the employee’s probationary period, once enrollment is complete and premiums have been paid.
Coverage ends if premiums are not paid, the person is no longer eligible, the policy ends, or the employee enters full-time active military duty.
Employees may enroll during Open Enrollment or after a qualifying life event.
Dependent coverage begins when the employee's coverage begins, unless the dependent is added at a later date due to a life event; in which case, dependent coverage becomes effective after the enrollment is approved and the premium is paid.
Dependent coverage ends when the employee’s coverage ends, or when the dependent is no longer eligible.
No. Enrollment is completed online. Franchise Benefits can assist if help is needed.
Invoices are billed one month ahead and go out on the 1st of each month with payment due by the 16th. The billing window closes on the 25th of each month. Any addition, deletion, or change entered after the 25th will appear on the following month's invoice. You have a 30-day grace period to submit payment. If payment is not received by the end of the grace period, coverage will be terminated as of the due date on the last unpaid invoice.
Payment is made online by credit card or ACH through our Bill & Pay system. You can pay as each invoice is rendered, or set up automatic payments. To set up autopay, you must first contact your FBS representative.
